Summary
Bottom line
RVx-201, also written RVx201, is one of three herpes vaccine candidates named by Rational Vaccines, and is its experimental live-attenuated therapeutic HSV candidate. The company currently describes its pipeline as investigational and preclinical.
The scientific lineage includes an HSV-2 mutant called 0ΔNLS and a preparation promoted as Theravax. People were exposed to related historical material outside regulator-authorized clinical research, but those uncontrolled experiments do not establish that RVx-201 is a clinical-stage program or that it is safe or effective.
No checked registered interventional study or primary human-results paper was identified through September 5, 2026. Two Rational-associated registry records are observational and did not administer the vaccine.
Plain-language overview
What is this?
RVx-201 is intended to use a weakened, replication-competent HSV-2 virus with changes involving infected cell protein 0, or ICP0, to stimulate a broad immune response.
Rational Vaccines presents it as therapeutic for HSV-1 and HSV-2. The identified lineage and checked primary preclinical studies are HSV-2 based, so the broader intended scope should not be confused with demonstrated benefit for either HSV type.
A 2021 paper explicitly called HSV-2 0ΔNLS 'RVx201,' while the current developer page calls 0ΔNLS a predecessor. Public evidence does not establish that the present candidate is compositionally unchanged from material used in older studies or human exposures.
Research goal
What were researchers trying to accomplish?
The developer's stated goal is a therapeutic vaccine that helps people with existing HSV better control disease after viral reactivation.
The checked evidence does not yet provide a regulator-authorized protocol defining the current candidate, dose, population, outcomes, safety monitoring, or statistical plan for an interventional human trial.
Study and program
What happened?
Mouse studies published in 2010 and 2011 examined attenuated HSV-2 ICP0 mutants, including 0ΔNLS. A 2021 guinea-pig study examined RVx201/0ΔNLS for 28 days and reported local, systemic, virologic, and immune observations.
An HHS Office for Human Research Protections determination letter records Southern Illinois University's finding that the investigator conducted human experiments involving himself and at least 15 other people between 2011 and 2014, with no institutional review-board review, FDA authorization, protocol, consent, or HIPAA documentation located in the institution's review.
In 2016, an uncontrolled effort in St Kitts reportedly enrolled 20 HSV-seropositive people for three injections; 17 completed the series. There was no randomized or placebo comparator, participants were asked to stop antiviral therapy, and no checked independently monitored clinical dataset was identified.
Rational later sponsored a completed UK questionnaire study, NCT05500053, and collaborated on observational NCT05823779. Neither study administered RVx-201 or another vaccine.
Reported findings
What did the study find?
The checked mouse studies reported attenuation, immune responses, and protection against HSV-2 challenge within their animal designs. The 2021 guinea-pig study reported short-term safety and immunogenicity observations but was not a therapeutic-efficacy study.
The 2016 investigator account reported fewer self-recorded symptomatic days after the injections and the company promoted a 'functional cure.' The uncontrolled design, antiviral withdrawal, self-reporting, incomplete participant accounting, and absence of independent monitoring prevent a reliable efficacy conclusion.
Public evidence is also inadequate for a reliable human safety conclusion. Sponsor reassurance and individual participant reports cannot replace a protocol-defined, complete safety dataset with an appropriate denominator and independent oversight.
HSVHQ interpretation
What does that mean?
The safest current evidence-maturity label is preclinical evidence. Historical administration to people did occur, but it was not a regulator-authorized clinical development program and cannot supply reliable controlled human evidence.
Animal protection, immune responses, absent short-term detection in selected samples, or theoretical interferon sensitivity do not establish human safety, efficacy, prevention of shedding, elimination of latent HSV, or cure.
Rational Vaccines says RVx201 received a UK Innovation Passport in 2021. Official UK guidance describes that pathway as development support, not trial authorization or marketing approval; HSVHQ did not independently match the exact candidate to a dated official award record in the checked routes.
RVx-201 is not established as approved or clinically available in the checked evidence.
Evidence gaps
What don't we know?
Program status
Why did or didn't development continue?
The developer currently lists RVx-201 as its lead HSV-2 candidate and explicitly describes the pipeline as investigational and preclinical.
The observational questionnaire and natural-history records do not change that stage because neither tested the vaccine. Historical unauthorized exposures are preserved as part of the record but are not treated as clinical-stage evidence.
Rational Vaccines holds a federal award running to December 31, 2026 for work toward an investigational new drug application, and that award funds a different candidate. Its abstract names RVx-1001, a preventive HSV-1 vaccine licensed from Louisiana State University, which the award record describes as engineered not to enter nerve fibres. No federally funded work, application or authorized trial has been identified for RVx-201 itself.
Confirmed next steps
What happens next?
The next evidence-changing milestone would be a regulator-authorized, registered interventional trial that clearly identifies the current formulation, protocol, population, outcomes, and safety oversight.
Before historical human claims could support a reliable conclusion, a complete independently reviewable dataset would need to resolve participant flow, adverse events, outcome ascertainment, antiviral use, and formulation identity. No such dataset was identified in the checked public record.

Timeline
OHRP issued oversight determination
The federal record documented serious noncompliance findings and limits on OHRP jurisdiction.
Why it matters: Provides the authoritative oversight boundary for the historical human experiments.
RVx201 guinea-pig study published
A 28-day preclinical safety and immunogenicity study identified HSV-2 0ΔNLS as RVx201.
Why it matters: Provides current-lineage animal evidence but no therapeutic human result.
Uncontrolled St Kitts human exposures
Twenty people were reported enrolled and 17 completed three injections without a randomized or placebo comparator or identified IRB/FDA authorization.
Why it matters: Human administration occurred, but the design and oversight failures prevent reliable clinical conclusions.
Technical record
Study details
The 2021 guinea-pig study gave 2×10^7 plaque-forming units of RVx201 intradermally to 16 female guinea pigs and intravaginally to three, with follow-up limited to 28 days. It assessed injection sites, clinical observations, antibody responses, and selected tissue and shedding assays.
The historical 2016 report describes three intradermal injections of 2×10^8 plaque-forming units. Because the present formulation is not publicly crosswalked to that material, HSVHQ does not treat the historical dose or outcomes as specifications for current RVx-201.
NCT05500053 enrolled 68 people for an observational questionnaire about genital-herpes symptoms and preferences for future trial procedures. NCT05823779 describes an observational cohort of 50 people with controlled HIV; its registry status is stale and no results are posted.
No published human trials are linked.
