Summary
Bottom line
GEN-003 was an investigational therapeutic vaccine that Genocea Biosciences tested in adults with recurrent genital HSV-2 infection. It reached Phase 2 and was never tested in Phase 3.
Across three randomized trials, genital HSV-2 shedding fell relative to each group's own pre-vaccination baseline. Genocea reported that its 131-person Phase 2b study met its primary shedding endpoint, at p=0.03 against baseline and p=0.05 against placebo.
At 12 months no dose differed significantly from placebo on shedding, and shedding had also fallen 52 percent in the placebo group. The 12-month lesion and recurrence results that did favor GEN-003 were secondary endpoints reported by the sponsor.
Genocea stopped spending on GEN-003 in September 2017 and attributed the decision to the time and cost of Phase 3, not to a safety problem or a missed endpoint. The candidate was never approved, and the company wound down in 2022.
Plain-language overview
What is this?
GEN-003 combined two laboratory-made HSV-2 proteins with an adjuvant called Matrix-M2. An adjuvant is a substance added to a vaccine to strengthen the immune response it produces.
The proteins were a transmembrane-deleted form of HSV-2 glycoprotein D and a large internal fragment of infected cell protein 4, both described in the trial records as T-cell antigens.
It was a therapeutic vaccine, meaning one given to people who already have the infection in order to reduce their own recurrent disease.
Research goal
What were researchers trying to accomplish?
The program aimed to reduce recurrent genital herpes disease in adults already living with HSV-2. Its main measured objective was genital HSV-2 shedding, meaning virus detectable by PCR testing on genital swabs that participants collected twice daily over 28-day periods.
Every trial enrolled HSV-2-seropositive adults with symptomatic recurrent genital herpes and excluded people with a history of genital HSV-1 infection. Genocea reported reduced recurrent lesions and shedding in HSV-2-infected guinea pigs beforehand, but those animal results did not establish benefit in people.
Study and program
What happened?
Six GEN-003 studies were registered in the United States between 2012 and 2017. Five were interventional trials at Phase 1/2 or Phase 2, and one was an observational follow-up study. The Phase 1/2a study randomized 143 adults from 2012 to 2014. A factorial Phase 2 dose-optimization study then randomized 310 adults from 2014 to 2016, with two companion studies: a Phase 2 rollover that re-randomized 37 former placebo recipients, and an observational study that followed 140 participants for longer without giving further doses.
The Phase 2b study randomized 131 adults from November 2015 and completed in May 2017. A 33-person maintenance-dose study began in May 2017 and was terminated; it is the only GEN-003 record with posted registry results.
On September 25, 2017 Genocea announced that it was ceasing GEN-003 spending and activities.
Reported findings
What did the study find?
In the Phase 1/2a study, whose registered primary outcome was safety rather than efficacy, shedding in the 30 µg group fell from 13.4 percent to 6.4 percent of swabs, with p<.001. That is a rate ratio of 0.47 against the group's own pre-vaccination baseline, while the 10 µg, antigen-only and placebo groups showed no reduction.
In the factorial Phase 2 study, shedding rate ratios against baseline ran from 0.45 to 0.82 across the six GEN-003 combinations, against 0.98 for placebo, in groups of 44 to 45 participants. At 12 months three combinations still showed significant reductions against baseline, including 0.38 at the 60/50 dose.
Lesion rates in that study fell immediately after dosing in every group, including placebo at a rate ratio of 0.38, which the investigators did not explain. Placebo recipients were then offered active vaccine, so no comparison against placebo was possible beyond 28 days after the third dose.
Genocea reported in September 2016 that the Phase 2b study met its primary endpoint, with shedding down 40 percent at the 60/50 dose at p=0.03 against baseline and p=0.05 against placebo, while shedding in the placebo group rose 6 percent. At 12 months it reported shedding down 42 percent at 60/50 and 52 percent in the placebo group, from about 30 participants per arm, with no dose significantly different from placebo.
The 12-month clinical secondary endpoints did separate from placebo in Genocea's report. Median lesion rate was 2.3 percent of days at 60/50 against 4.5 percent with placebo, and median recurrences were 1.5 against 4.0, each with p=0.01, in a trial of roughly 43 to 44 participants per arm.
Those were secondary endpoints reported by the sponsor. Only the abstract of the peer-reviewed Phase 2b report was accessible in the checked sources, so that trial's confidence intervals, analysis populations and prespecified comparisons cannot be independently described here.
Genocea and the trial investigators reported no treatment-related serious adverse event and no death across the Phase 2, Phase 2b and maintenance studies. These are sponsor and investigator reports covering the 474 participants randomized in those three studies, not evidence that GEN-003 was proven broadly safe.
HSVHQ interpretation
What does that mean?
GEN-003 generated controlled human efficacy evidence, meaning randomized placebo-controlled trials measuring virologic and clinical outcomes in people. It never generated large confirmatory evidence, because the Phase 3 program never started.
Almost every effect estimate in the record compares a group against its own pre-vaccination baseline rather than against placebo, and placebo groups either fell in parallel or were not followed. The checked evidence therefore establishes biological activity on a virologic measure more firmly than it establishes placebo-controlled clinical benefit.
HSVHQ records this program as discontinued rather than failed. The dose-optimization and Phase 2b trials were reported as meeting their primary shedding objectives, the one posted null primary came from the terminated maintenance sub-study whose own record says it likely lacked the power to detect a difference, and the developer gave a financial and strategic reason for stopping. That is also not the same as saying GEN-003 was shown to work and then abandoned, because the confirmatory trial that would have tested the sponsor's claims at scale never ran.
Reducing detectable shedding is not the same as reducing transmission, and no GEN-003 study measured transmission to a partner. Immune responses were induced, but none was shown to predict who benefited.
Evidence gaps
What don't we know?
Program status
Why did or didn't development continue?
GEN-003 is discontinued. On September 25, 2017 Genocea announced a strategic shift to immuno-oncology, described GEN-003 as its Phase 3-ready investigational immunotherapy, and said it was ceasing GEN-003 spending and activities while reducing its workforce by approximately 40 percent.
Management slides filed two days later gave the reason: positive 12-month Phase 2b data had not led to a pathway for Genocea to independently advance GEN-003, given the time and cost of Phase 3 trials. Positive is the company's characterization of its own data. The registry record for the terminated maintenance study describes the stop as a business decision unrelated to product safety, and no checked source attributes it to a safety signal or a failed efficacy endpoint.
In May 2020 Genocea transferred certain HSV-2 antigens from the program to Shionogi and Co., Ltd. under a material transfer agreement carrying an exclusive option to negotiate a license. According to Genocea's 2021 annual report, Shionogi allowed that option to lapse in July 2021, due to a change in Shionogi's corporate focus.
Genocea's board voted on May 20, 2022 to wind down operations, and the company suspended its SEC reporting that June. GEN-003 was never approved, was never offered through expanded access, and completed no study beyond Phase 2. No current owner or developer of the program is recorded in the checked sources.
Confirmed next steps
What happens next?
No confirmed development milestone was identified, and no organization has been identified that could resume GEN-003. Genocea ceased public reporting in June 2022, so no newer statement from the original developer can exist.
This record should be updated if the full peer-reviewed Phase 2b report becomes available with placebo-controlled estimates, if results for the rollover or long-term follow-up studies are published, or if a public record establishes who holds the GEN-003 patents or antigen materials.

Timeline
Genocea voted to wind down operations
The board voted to wind down after a strategic process did not identify a viable going-concern transaction; SEC reporting was suspended on 2022-06-13.
Why it matters: No current owner or developer of GEN-003 is recorded after this point.
Shionogi option over the antigens lapsed
Genocea reported that Shionogi allowed its exclusive option to license the GEN-003 antigens to lapse, citing a change in Shionogi's corporate focus.
Why it matters: Closes the only identified successor path for the program's antigens.
Genocea ceased GEN-003 spending
The company announced a strategic shift to immuno-oncology, stopped GEN-003 spending and activities, and reduced its workforce by about 40 percent.
Why it matters: Establishes the discontinued status and its stated financial and strategic basis rather than a safety or efficacy failure.
Phase 2b 12-month results reported
Genocea reported reduced lesion days and recurrences at the 60/50 dose, while no dose differed significantly from placebo on shedding at 12 months.
Why it matters: Separates the secondary clinical signal from the shedding endpoint, which had converged with placebo.
Phase 2b primary endpoint reported as met
Genocea reported a 40 percent reduction in genital HSV-2 shedding at the 60/50 dose, at p=0.03 against baseline and p=0.05 against placebo.
Why it matters: This is the program's strongest sponsor-reported primary result, and it is a shedding measure rather than a clinical outcome.
Technical record
Study details
Each interventional trial gave three intramuscular doses 21 days apart, except the maintenance study, which gave a single dose to people who had completed the Phase 2b study. The observational follow-up study gave no doses. The factorial Phase 2 study tested 30 or 60 µg of each antigen with 25, 50 or 75 µg of Matrix-M2, written here as antigen dose over adjuvant dose.
The Phase 2b study compared 60/50 and 60/75 with saline placebo, using a new formulation manufactured with commercially scalable processes. The checked sources do not describe what changed beyond the manufacturing process.
Reactogenicity was common and associated with the adjuvant. At least 98 percent of GEN-003 recipients in the factorial Phase 2 study reported it, against 62 percent of placebo recipients.
GEN-003 raised antigen-binding IgG, neutralizing antibody and T-cell responses, some of which persisted to 12 months. T-cell responses in the Phase 1/2a study did not correlate with shedding or lesion reduction, and the investigators stated that a correlate of protection, meaning an immune measure that predicts clinical benefit, had not been defined.
